Certified Peer Support Services

  • Our unique engagement model puts peer supports at the front of support system. We use personal lived experience as an evidenced-based practice that increase outcomes and decreases relapse.

Employment Support and Coaching

  • A large focus of our programs is teaching each person the skills needed to maintain independence and employment is key. We connect members to job training and teach skills needed for competitive employment

Collaborative Care Coordination

  • With our collaborative support each member works directly with a Care Coordinator to set goals, get connecting to resources and wrap them with supports during each transition within our IOP or supportive programs.

One-on-One Coaching and Mentoring

  • We can’t do it alone! This is our motto for everything we do. What this looks like in practice is utilizing mentorship programing, staff mentorship and creating an environment that is safe and supportive.

Resource Support and Connection

  • As our members transition from different levels of care, we have a large base of collaborative partners and resources we utilize to ensure our members have wrap around supportive services that support successful independence.

We evaluate individuals using unique, innovative, and dynamic assessments to determine the appropriate level of care based upon their individuals need.
